MedChart is not a medical device and does not provide diagnosis, treatment, emergency triage, or professional medical advice.

You confirm that you are at least 18 years old and have legal capacity to create an account and provide information in MedChart.

You agree not to misuse MedChart, interfere with its operation, attempt unauthorized access, upload unlawful content, or use the service in a way that violates applicable laws or the rights of others.
